The CAC waiver funds residential and in-home care for Minnesotans who are chronically ill or medically fragile. See who qualifies and find providers.
About This Waiver
The CAC waiver (Community Alternative Care) is a Minnesota Medical Assistance waiver for people who are chronically ill or medically fragile and would otherwise require hospital level of care. It funds intensive home and community-based services — in-home nursing-level supports, adult foster care, and 245D-licensed residential settings equipped for complex medical needs. CAC serves both children and adults, and placements typically require providers with strong medical staffing.
CAC serves people of any age who are chronically ill or medically fragile and need the level of care provided in a hospital. Eligibility is determined through a MnCHOICES assessment with medical documentation.

Find Providers
Open a care type to browse licensed providers by county and see openings when providers update capacity.
Adult foster care provides licensed residential support in family-home settings, typically serving one to four people. Caregivers live on-site or provide around-the-clock presence in a more personal environment than larger group homes. Homes are licensed by the Minnesota Department of Human Services. Capacity Track lists adult foster care by county and shows openings when providers update.
Community Residential Setting (CRS) group homes provide 24-hour residential support for adults with disabilities, brain injuries, and mental health conditions. Licensed under Minnesota’s 245D framework by the Department of Human Services, CRS is one of the most requested placement types for waiver-funded services. Capacity Track lists licensed CRS providers statewide and shows openings when claimed providers update capacity.
Choose the care type and county, describe the needs once, and we email every matching licensed provider that takes email inquiries. You get a list of the ones who reply. No names or medical details needed.

Common Questions
CAC funds intensive community-based supports for hospital-level needs: in-home services, adult foster care, and residential settings able to support complex medical care — along with equipment, modifications, and respite. Room and board is covered separately from waiver services.
Because CAC placements need providers with medical staffing, start with county pages for adult foster care and CRS, then confirm nursing coverage and medical capability directly when you inquire.
