About This Waiver
The CADI waiver (Community Access for Disability Inclusion) is a Minnesota Medical Assistance waiver for children and adults with disabilities who would otherwise require nursing-facility level of care. It funds home and community-based services so people can live in the community — including residential supports in 245D-licensed settings, customized living in assisted living communities, adult foster care, in-home services, and day supports. CADI is one of the most common funding sources case managers work with when searching for residential openings.
CADI serves people of any age who have a certified disability, need the level of care provided in a nursing facility, and can live in the community with waiver supports. Eligibility is determined through a MnCHOICES assessment coordinated by the county, tribal nation, or managed care organization.
