About This Waiver
Customized living is the Minnesota waiver service that pays for care in assisted living. It is an individualized package of health-related and supportive services — personal care, medication management, meals, homemaking — delivered in a licensed assisted living facility and funded under the Elderly Waiver (EW), CADI, or BI waivers. When it includes round-the-clock supervision, it is authorized as 24-hour customized living. If a case manager or care coordinator says a client "needs a customized living setting," they are looking for exactly the providers listed here.
Customized living serves adults 18 or older on the EW, CADI, or BI waivers who live in a qualified assisted living setting. 24-hour customized living additionally requires an assessed case mix of A through K or V. Rates and service plans are set by the lead agency — the county or Tribal Nation case manager for fee-for-service, or the MSHO / MSC+ care coordinator for people in managed care.

A family shopping privately looks for "assisted living" — a case manager placing a waiver client authorizes "customized living" in that same setting. Not every assisted living community is enrolled to provide customized living, so confirm waiver acceptance when you inquire.
Customized living with 24-hour on-site supervision, for people whose assessed needs require staff around the clock. Eligibility requires a case mix of A through K or V (case mix L does not qualify). Memory care under the waivers is typically authorized as 24-hour customized living in a dementia care unit.

Adult foster care provides licensed residential support in family-home settings, typically serving one to four people. Caregivers live on-site or provide around-the-clock presence in a more personal environment than larger group homes. Homes are licensed by the Minnesota Department of Human Services. The Elderly Waiver (65+), CADI, and BI waivers. Note the age rule for CADI and BI: customized living settings that enrolled with DHS on or after January 11, 2021 may only serve CADI and BI clients who are 55 or older — the authorization is blocked in MMIS for younger clients. Clients under 55 need a setting enrolled before that date, or a different residential service such as CRS or adult foster care.
No. The waiver pays for services only. Rent and meals come from the resident’s income — SSI, Social Security, pension — often with help from Housing Support (formerly GRH), which pays room and board for eligible low-income residents in assisted living, adult foster care, and board and lodge settings.
The lead agency. For people on fee-for-service waivers, that is the county or Tribal Nation case manager. For the majority of Elderly Waiver participants — who are enrolled in MSHO or MSC+ managed care — the health plan’s care coordinator completes the assessment, sets the rate with the EW Residential Services tool, and authorizes the placement.
