Oregon Health Plan CCO managed care covers in-home and community services only — it does not pay for residential placement in Oregon. The program serves general Medicaid.
Enrollment and service authorization go through your services coordinator, typically via your Community Developmental Disabilities Program (CDDP). They confirm eligibility, authorize the budget, and help select a provider.
Oregon Health Plan CCO managed care does not fund residential placements. In Oregon, residential care can be funded through Community First Choice State Plan (K Plan, 1915(k)), APD Aged & Physically Disabled 1915(c) waiver, I/DD Comprehensive and Support Services waivers (1915(c)), and others — talk to your services coordinator about switching or adding a program.
