Enrollment and service authorization go through your case manager or support coordinator. They confirm eligibility, authorize the budget, and help select a provider.
Family Support 360 waiver does not fund residential placements. In South Dakota, residential care can be funded through HOPE waiver (Home & Community-Based Options and Person-Centered Excellence), CHOICES waiver — talk to your case manager or support coordinator about switching or adding a program.
