How to Place Someone in Assisted Living
Confirm Who Pays
Private pay, the Elderly Waiver at 65 and older, or CADI or BI under 65. People on MSHO or MSC+ get the Elderly Waiver through their health plan care coordinator. Waiver moves wait on a MnCHOICES assessment.
Name the Level of Care
Memory care needs a setting with an assisted living with dementia care license. Someone who needs supervision around the clock may qualify for 24-hour customized living under the waiver.
Send One Request
Give the county, care needs, funding, and move date. Every matching provider that takes email inquiries gets it at once, instead of one phone call at a time.
Confirm Before Discharge
Providers who reply can confirm the rate, the service plan, and the move-in date. A reply is not guaranteed.
What Providers Ask for in a Referral
- Diagnoses, current medications, and who manages them
- Mobility, transfers, and any recent falls
- Memory, behaviors, and whether the person might leave unsafely
- Funding: private pay, Elderly Waiver, CADI, or BI, and the case manager or care coordinator
- MnCHOICES assessment date and case mix, if on a waiver
- Where the person is now (hospital, TCU, nursing home, or home) and the target move date
The request itself collects no names or health records. Share those only with the provider you choose.

Common Questions
Frequently Asked Questions
How do I find assisted living openings in Minnesota?
Most providers do not publish openings, and they change week to week. Providers who update their listing appear in the open beds list above. For the rest, send one request: matching providers that take email inquiries receive it, and you hear back from the ones who reply.
Who pays for assisted living placement in Minnesota?
The resident pays privately, or a waiver pays for the care: the Elderly Waiver at 65 and older, or CADI or BI under 65. MSHO and MSC+ members get the Elderly Waiver through their health plan. Room and board is separate, and Housing Support can help when income is low.
Does any assisted living accept the Elderly Waiver?
No. The provider must be enrolled to deliver customized living and willing to accept the waiver rate. Put the funding in the request so providers know before they reply.
Is there a fee to send a placement request?
No. Sending a request is free for families, case managers, and discharge planners.
