Common Questions

Frequently Asked Questions

What are the Elderly Waiver customized living rates for 2026?

Effective October 1, 2026, monthly limits for customized living run from $3,081 (case mix A) to $7,164 (case mix K). For 24-hour customized living they run from $5,483 to $13,450. DHS publishes them on form DHS-3945.

What is a case mix?

A letter assigned in the MnCHOICES assessment that reflects how much help someone needs. It sets the monthly ceiling for their waiver services. From A through K the ceiling rises with need; L is for lower needs and V is for people who are ventilator dependent.

What is the difference between customized living and 24-hour customized living?

24-hour customized living is for people who need supervision available around the clock, which is common in memory care. It carries a higher monthly limit than standard customized living.

Does the Elderly Waiver pay for room and board?

No. The waiver pays only for care and services. The resident pays room and board from income; when income is low, Housing Support can pay up to $1,221 a month toward it in assisted living (effective July 1, 2026).

Is the limit what the provider will be paid?

Not necessarily. The limit is a ceiling. The case manager builds the actual rate from the person’s service plan, and it can come in below the limit.
