HCBS-SLS (Supported Living Services) waiver covers in-home and community services only — it does not pay for residential placement in Colorado. The program serves ID/DD living in own/family home.
Enrollment and service authorization go through your case manager, typically via your Case Management Agency (CMA). They confirm eligibility, authorize the budget, and help select a provider.
HCBS-SLS (Supported Living Services) waiver does not fund residential placements. In Colorado, residential care can be funded through HCBS-EBD (Elderly, Blind and Disabled) waiver, HCBS-DD (Developmental Disabilities, comprehensive) waiver, HCBS-BI (Brain Injury) waiver — talk to your case manager about switching or adding a program.
