Enrollment and service authorization go through your case manager or support coordinator. They confirm eligibility, authorize the budget, and help select a provider.
Model II Waiver does not fund residential placements. In Kentucky, residential care can be funded through Supports for Community Living (SCL), Acquired Brain Injury (ABI) and ABI Long-Term Care — talk to your case manager or support coordinator about switching or adding a program.
